‘Care Partners’ refer to the personal support team of a patient receiving medical care. They volunteer for their loved ones, and have the ability to complete tasks with them or on their behalf.
‘Care Managers’ are skilled nurses or social workers that can be hired by Sandwych to fill Care Partner gaps for families.
SliceAssist is the powerful AI engine inside of the Sandwych App that automatically converts a physician’s orders and care plans into trackable tasks. To complete those tasks, SliceAssist will use behavioral science to provide nudges, notifications, and prompts to the patient and the members of their personal Care Team who are strategically located, available, and willing to do so.
When members of the care team are not available, SliceAssist will also serve up options for local business & organizations (aka Human Helpers) who can fill in care gaps.
Providers are your primary care physicians, pharmacists, mental health professionals, and anyone else providing expert care or services to someone in need.
A stand-alone entity with a valid EIN serving within one of the seven components of human health: Environmental, Physical, Emotional, Intellectual, Social, Spiritual, and Financial.
