Through our EHR integration, physicians can easily share an invitation to Sandwych while inside a patient’s records. With that invitation, patients can enjoy all Sandwych has to offer for FREE.
Allowing them access to this app will simplify both care navigation and coordination, all while providing the emotional support and appropriate resources they’ll need to be successful in their health journey.

Once enrolled, users can add family members, neighbors, and friends to their Care Team (as either an intimate Care Partner or as part of their informal Social Circle).
Of the 5 assessments they’ll be required to complete, one captures & documents SDOH and risk behaviors that can be barriers to care. This type of documentation will be powerful in helping physician’s understand how to approach the patient in the future.

Prior to appointments, providers can request an updated medication history.
After appointments, providers enter patient care plans and referrals into Sandwych via EHR, and a series of trackable tasks are auto-generated & documented. The SliceAssist smart engine offers triggers, nudges & notifications to keep patients on task.

Based on patient preferences and Care Team availability, Sandwych will offer up local referrals by utilizing the built-in Local Resources database via SliceAssist. Providers can review connections to track referral acceptance and adoption for a better understanding of the community’s active network of support.
The Sandwych Resource Stack is full of articles with caregiving tools and topics such as obtaining legal documents, housing and care options, maintaining independence, and how to discuss life changes.

Sandwych offers assistance for both patients and care partners, and automatically handles a large number of provider administrative tasks.
